1. For a concrete slab downstaris: Would you use glue down engineered or 3/8 solid with wood underlayment?

2. Based on the answer above, would you glue down 3/8 solid upstairs (over wood subfloor) or would you nail down 3/4 solid?

(In the questions above, cost and functionality are both considerations.)

3. Can you recommend no or low formaldehyde engineered brands?

I know that Kahrs was an earlier answer, but I was wondering if there were others.

4. Austin Floor Guy: Know any good folks at the $3/sqft labor range in the Woodlands, Texas area?

engineered for downstairs ( you will have more wood on it for future refinishings ect

3/4 solid nailed upstairs (more cost effective)

I would use an engineered, seen too many solids over concrete have serious issues

What ever it is, I would fasten it over the wood substrate.

As long as it isn't from China, there should not be a concern with formaldehyde.
