I'm putting down 3/4" maple over concrete with 3/4" plywood under the wood floor. What should I do for the transitions to stained concrete?

I was thinking of plaining the back side off to glue a few together and make a custom transition beveled at 45 degrees. Are there any better ways of doing it? I have 2 going to the kitchen, 2 going into bathrooms to deal with.
