I am using 5" wide, 3/4" thick, T&G white oak over contractor grade, 3/4" T&G OSB. There is radiant heat under the flooring. Do I need to glue to avoid cupping due to the width of the flooring?

Solid 5" over radient heat. You can glue and fasten the planks, but IMO, that is a mis-specified flooring to go over radient heat. Expect gaps and cracks as the solid wood dries out.

Cupping is going to be the least of your worries, unless the substrate is wet.

What about an underlayment instead of gluing?

I still would imagine you're asking for trouble. Your product just really isn't the right type to be put over radiant heat.
