 Post subject: Transitions

After a search on "transitions" I found a few helping posts but still have a couple lingering questions. I'm putting down a 3/4" wood floor on top of 3/4" plywood on top of a concrete slab. I have two sets of French Patio doors that swing out onto a lanai. How do I transition to the doors? The floor strips will butt against the doors. #2, from the bedroom to the bath I'll be switching from wood to tile, I hadn't thought about putting the subfloor in the bath and was going to tile on top of the slab but that will leave 1 1/2" (minus the thickness of the tile) between the bedroom and bathroom! Any suggestions sure would be appreciated.

It's best to build up the tile subfloor(if possible) so both have the same vertical height. Otherwise custom made thresholds are needed.
