take a 5 or 6 inch wide board for the transition ( you will need to rip it down to the width of the doorway, but without knowing the angle I cant say how wide to rip it, standar doorways are 4.5 inches, but if you ramp it up it may need to be closer to 6 inches.) put it where it should go (in the doorway or whatnot) cut eack side of the plank at 10-15 degree angle inward (it may wind up being more to accomidate the hieght) \_/ you can then route a grove in each side and lock both sides of the floor into it.
We usually run a strip of wood along the higher part to brace it we cut the top of the board to match the angle of incline for support. The front lower part we shoot a couple face nails on a 45 through the grove, glue the front and back of the board to everything, the subfloor, and a little yellow glue in the t&g.
If you are sanding the floor just go up to it with the edger and palmsand the threshold smooth.
If its prefinished a quick run of the round part on a chisel will make a decent micro beveld edge.
we have done several of these to go up to tile with heated bases and thick tile, I think the highest we went was 2 inches
I only have pictures of a small one we did was just under 3/4 higher, with a 5 inch oskkosh piece. The wider the board, the smoother the transition.
