Yes I put underlayment down and floor is flat. I have decided to put 3/4 plywood down and then nail. A friend is loaning me a nailer. I just have to locate a 5/8 shoe/plate. My question is: How do I anchor the plywood to the concrete? Ramset? Adhesive? or both?. When I get close to the wall and the nailer won't fit. (elementary question I know) but what is the industry way of attaching the last runs? Glue to subfoor?, or nail and fill?. Thanks in advance, ....The neophyte

Depends on the size of the rips. If they are an inch or larger just go ahead and pull them tight with a crow bar and nail them as close to the wall/base as possible to avoid having to fill them. If they are less than an inch glue the groove and wedge them in or use blue tape to close them whatever is easiest.
