what is the righ way to and install plywood over concrete for hardwood floor installation

Install a floating sub-floor by screwing together two layers of plywood.

6 mil polyethylene plastic,

then plywood( preferred underlayment) shot into slab

install sheets opposite to the preferred floor direction..

cover with felt or the like and install flooring with 11/2" nails/staples

Go with the 6 mil poly and then use the two layers of plywood. No sense putting a moisture barrier down then shooting it full of holes IMO.

Go with a trowel on vapor retarder, either roofing mastic and 6 mil or Bostik MVP4 and 6 mil then float the ply. I agree with Mickey, less holes less potential for failure.

You can glue 1/2'' to 3/4'' thick strips of wood straight to the concrete and then install your wood floor on top of that.
