 Post subject: subfloor over concrete

I would like to install a 3/4" hardwood floor over a concrete slab. I have been told a 3/4'" plywood subfloor should be installed to nail to. Do I need some type of vapor barrier under the plywood? What is the suggested method of fastening the ply to the concrete? Is there a recommended nailing pattern for the plywood to the concrete? Thanks for any suggestions.
